Employees’ Compensation Claims – Amenities & Operations – Our District

Each Berkley School District employee is asked to fill out an employee injury report form for any injury (even if medical care is not required).

It is imperative that this form is completed, signed by the property manager, and forwarded to Barb Woolery at Avery as soon as possible after the incident. Information must be sent to our insurance carrier in good time. Make sure you provide as much information as possible (what the employee did just prior to the incident, details of the incident, time and location of the injury) to expedite filing a claim if necessary.
